If you have currently have or are in treatment for Breast Cancer or HIV/AIDS then you qualify for our client services.
Our services require that you complete our intake process which takes roughly an hour, of which some parts can be completed through the mail.
Our services are listed on the top panel. Click through them to see if we can take care of your needs and drop us an e-mail at clientservices@breakingbarriers-sacramento.org if you believe there is a service that the community needs. We love to hear feedback!
Breaking Barriers has limited resources for food. The food that we supply to our clients comes from the River City Cmmunty Services and private donations.
To qualify for our food program, you must complete a Food Intake with a staff person, who will score you in several important areas:
your income
the number of people (especially dependant children) in your household
any other disabilities you or your primary caregiver may have
your ability to get to the grocery store
the relative closeness of a food closet.
Our food program does have a waiting list. Average wait before becoming active is 2 months.
We can help with appointments for your Doctor, Case Manager, Social Worker, Dentist, Counselor, Psychiatrist, Court Dates, Clinic Appointments, and if we have the people available, support group meetings, grocery shopping and special events.Call us as soon as you know about your appointment to get it on the calendar.
We give priority to appointments in the following order: Medical/Dental, Case Management/Social Work, Counseling/Psychiatry, Support Group/Court Dates, anything else.
Because of the need for daytime drivers for medical and case management appointments, we cannot provide rides to the grocery store during the hours of 7:00AM and 7:00PM
If you have something that you need a ride to and you aren't sure if it qualifies, just ask us!
In order for us to do our best, and to provide you with the most reliable service, we need to know about upcoming appointments as soon as possible.
It generally takes us at least 2 business days to contact enough volunteers to find someone with an open time slot. If we know about your appointment with at least 2 days notice, we can generally get it done.
When we find out about an appointment or ride with less than 2 days notice, we often don't have enough time to help.
We have volunteers who enjoy helping out people in the home. These volunteers are available in teams of two or more, to help with light housekeeping duties like vacuuming, moving furniture, cleaning the bathroom, dusting and general cleaning-up.
A staff person will come out to your home and make an onsite assessment of your needs before we schedule any volunteers to come out. This service can be arranged to be ongoing. Volunteers can be scheduled in advance for weeks and months at a time to make sure someone comes out to help you on a regular basis.
We need at least a week's advance notice of household help to get the volunteers arranged.
Through our grant with Susan G. Komen we are able to provide our clients with Breast Cancer with case management services to ensure their success in treatment, this is also largely accomplished through the hard work of our SNAP volunteers!
For our clients with HIV/AIDS we refer out to CARES, HRS and Maap, Inc. for case management depending on need.
